Federal Tort Claims Act Medical Malpractice Coverage

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).

HIPAA Privacy Policy

It is the intent of HCCHC to comply with all rules mandated by the new Health Insurance Portability and Accountability Act (HIPAA) Omnibus Update and upgraded by Health Insurance Technology for Economic and Clinical Health Act (HITECH), which reflected changes to patient’s rights and other issues involving the privacy and security of protected health information (PHI) and any future updates which shall be made at a future time.

Certified Application Counselors (CAC) & Designated Organization (CACDO) related to Affordable Care Act

HCCHC has signed an Agreement with the Centers for Medicare & Medicaid Services to be a Certified Application Counselor Designated Organization (CACDO). HCCHC has signed Agreements with selected staff members to perform duties as Certified Application Counselors (CACs). These CACs have completed required training and may assist consumers, applicants, and individuals who are interested to finding out about and possibly applying for the State’s Federally-Facilitated Health Insurance Marketplace or Exchange. The organization shall abide by the standards and requirements pursuant to 45 CFR 155.225, and shall abide by the applicable Privacy and Security Standards 45 CFR 155.260

Title VI Compliance

HCCHC complies with Title VI which prohibits discrimination on the basis of race, color, or national origin in program and activities receiving federal financial assistance. Specifically, Title VI provides that “no person in the United States shall, on the grounds of race, color, or national origin, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity receiving Federal financial assistance” (42 U.S.C. Section 2000d).

DHHS Grant

We receive a Consolidated Health Services Federal Grant from the US Department of Health and Human Services (DHHS) that helps provide some services at a discount for qualified patients. The CHC grant funds make up a portion of our budget. We are an equal opportunity employer.
